For parents and caregivers of youth with disabilities and youth with autism who are registered in the Ontario Autism Program (OAP).

What to Expect

The High School Transition Kit is back! In collaboration with the Horizons youth program at TVCC, Foundational Family Services is offering a High School Transition Kit filled with resources, information and tools to help parents and teens prepare for the transition into high school.

The kit includes:

  • "About Me" template
  • High School Timeline and Planning worksheet
  • Planning for High School Bingo Card
  • "Getting involved" Social Narrative
  • Resource List
  • and Positive Affirmation or Inspirational Word Stickers!

**Kits offered through Foundational Family Service are for families of children/youth at TVCC. If your child/youth is registered in the Ontario Autism Program (OAP), you will be asked to confirm your child's registration in the OAP and to provide an OAP Client Information Reference Number if you have received one.**
